For some children, however, the tonsils become a source of problems rather than protection. They can cause repeated sore throats, disturbed sleep due to snoring or obstructive sleep apnoea, or problems with swallowing. In these situations, surgery may be advised. Two different procedures are commonly discussed: tonsillectomy and tonsillotomy.

What is a Tonsillectomy?

A tonsillectomy means the complete removal of the tonsils. This has been a standard operation for many decades and is still widely used, especially if a child has frequent tonsillitis (repeated throat infections). Removing the tonsils completely means they cannot grow back or cause further infection. However, because the whole tonsil is taken out, the operation can cause more throat pain afterwards, sometimes lasting up to two weeks. There is also a small risk of bleeding during the recovery period, which is why children need to be carefully monitored after the operation.

What is a Tonsillotomy?

A tonsillotomy is a more modern technique where only part of the tonsils is removed. This is sometimes called “partial tonsil removal” or “tonsil trimming.” It is mainly used for children who have large tonsils causing snoring or sleep apnoea, rather than repeated infections. Because some tonsil tissue is left behind, recovery tends to be quicker, with less pain and a lower risk of bleeding compared to a full tonsillectomy. Children are usually able to return to normal eating and activities sooner. Very occasionally, the tonsil tissue can grow back and cause problems again, but this is uncommon.

Which operation is best?

The decision between tonsillectomy and tonsillotomy depends on the main problem your child is experiencing.

If the issue is recurrent tonsillitis in an older child or adult, then a full tonsillectomy is sometimes recommended.

If the main problem is snoring or obstructive sleep apnoea, or the child is less than 5 years old then a tonsillotomy may be the preferred option as it is less painful and has a smoother recovery.

Mr Ramsden’s role

As a paediatric ENT surgeon in Oxford, Mr James Ramsden has wide experience in both techniques.
